From CMS GUIDE goals to a practical Companion experience

CMS created the GUIDE model to strengthen dementia care navigation, support unpaid caregivers, and expand access to respite and community services while people remain safely at home.

GUIDE Companion translates those high-level requirements into a day to day experience: clear navigation, plain language, and structured touch points that are easy for families, nurse practitioners, and community partners to use.

When a GUIDE family uses their respite allowance at an adult day center, the center’s side of the day runs on the Day Center Companion: intake, attendance, a staff-approved evening note, and a closeout record the partnering GUIDE program can reconcile. Respite is paid through the participating GUIDE program; the allowance belongs to the family.

Aligning steps in GUIDE projects

How each GUIDE Companion project phase supports CMS expectations

Every GUIDE project begins with a discovery and alignment phase, moves through a preview period, and grows into pilot and scale up. Each step is shaped by the CMS GUIDE model so navigation, respite, and caregiver support requirements are visible from the start.

  • Discovery: map existing dementia services, caregiver pain points, and GUIDE registry plans so the Companion reflects real workflows.
  • Preview design: configure tiles, pathways, and documentation prompts that match GUIDE care management, respite, and caregiver support expectations.
  • Pilot and measurement: follow a small cohort of GUIDE families, watching for stress, safety, and respite usage signals that can be shared with quality teams.
  • Scale and refinement: expand across clinics and community partners while tuning prompts, Care Codes (a code you scan or a link you tap), and documentation so they stay aligned with CMS guidance as it evolves.

Research threads behind GUIDE Companion design

GUIDE Companion weaves together research from dementia care navigation, caregiver burden, respite and adult day programs, and community-based dementia services.

Instead of presenting studies directly in the Companion, we bake their lessons into layout, language, nudges, and pathways so the experience feels simple even when the science underneath is complex.

  • Caregiver burden and stress research that supports early outreach, regular check ins, and coaching on coping skills.
  • Evidence that adult day and respite services are linked to better caregiver well being when families can find and use them; the studies we have do not support the claim that adult day delays residential placement.
  • Studies on SMS and chat based coaching that show how short, actionable messages improve follow through on safety, appointments, and medication routines.
  • Behavioral science work, including carePhysics, that informs tone, sequencing, and the way the Companion explains choices and next steps.

Learning alongside GUIDE teams

Closing the loop between CMS metrics, research, and real families

GUIDE Companion is not a static product. Each implementation becomes another opportunity to learn with GUIDE teams, refine pathways, and share back what is working across programs and communities.

Over time, GUIDE partners contribute de identified stories, metrics, and qualitative insights that shape future versions of the Companion and the broader research agenda.

The adult day and respite side of that loop has its own evidence base. Day Centers Inspire, our community white paper and research report for adult day centers, grades every claim on a ladder: service days relieving caregivers is supported by daily-diary research, cost savings are unproven, and delayed residential placement is a claim the studies we have do not support. GUIDE teams referring families to adult day can read the whole ladder and the funding map, with dated figures, on the Day Center research page.

  • Structured feedback after calls, chats, and respite referrals that highlights which supports feel most valuable to caregivers.
  • Dashboards and summary views that help GUIDE leaders see patterns in navigation, respite usage, and caregiver stress markers.
  • Links to deeper reading on the Studies and Research Collection pages so clinical, quality, and research teams can explore primary sources.
  • A roadmap that keeps future CMS guidance, community innovations, and new research findings in view so GUIDE Companion can evolve with the model.
  • Adult day and respite evidence from Day Centers Inspire: service days are linked to fewer care-related stressors and better caregiver affect (Zarit et al., 2014), and placement effects remain an open question (Udeh and Menne, 2025).
  • A shared closeout record: GUIDE families booked as adult day reservations appear on the center’s closeout so the partnering GUIDE program can reconcile covered days. Respite metering stays with the program.
